Privacy Policy
This website provides information about non-surgical knee pain care in the Milwaukee area, offered through Third Coast Vascular in partnership with Vascular & Interventional Specialists (VIS) and its care team. This policy explains what information this website collects, how it is used, and the choices you have. It covers this website's data practices; if you become a patient, the care team's HIPAA Notice of Privacy Practices governs how your protected health information is handled in the course of your care.
Information you provide
When you submit the request form, you provide your name, phone number, email address, and ZIP code, and (optionally) your consent to be contacted. The form is hosted by Jotform under a HIPAA-compliant Business Associate Agreement (BAA), and submissions are encrypted in transit and at rest. We use this information to contact you about your request, review your options and your coverage, and schedule care if you choose to move forward.
Phone calls
When you call a number shown on this website, the call may be routed through CallRail, a call-tracking service that helps us understand which ads and pages lead to calls. The number you see may change based on how you reached the site; the underlying office line and destination stay the same. Calls placed through this website are not recorded.
Information collected automatically
Like most websites, this site collects certain technical and usage data through cookies and similar technologies, such as IP address, browser and device type, pages visited, and how you arrived at the site (including ad click identifiers). We use this to measure ad performance and improve the site.
These measurement tools are managed through Google Tag Manager and include:
- Google Analytics 4 — website traffic and conversion measurement.
- Microsoft Clarity — anonymized, masked usage insights.
- Google Ads — conversion tracking for search and display ads.
- Meta (Facebook/Instagram) pixel — conversion measurement for social ads, configured with restricted data collection.
How your information is shared
We do not sell your personal information. We share information only with the service providers described above so they can perform services on our behalf, with the care team so they can follow up on your request, and where required by law.
Your choices
Providing your information and consenting to contact are always optional. You can opt out of text messages at any time by replying STOP; see our SMS Terms for details. You can also ask us to access, correct, or delete the information you have submitted, or ask a question about this policy, by calling us at (414) 348-4886.
Changes to this policy
We may update this policy from time to time. Material changes will be reflected by updating the effective date above.
